Job Description:

This professional position provides care coordination services to meet medical, behavioral health and social needs of Independent Care members in Milwaukee County while promoting and maintaining the quality of care for members.

  1. Conducts initial and annual assessments of members’ medical, behavioral health and social needs.
  2. Coordinates and organizes information from members assessments to develop a care plan that addresses their needs and assures implemented interventions are collaborated with members and their providers/supports.
  3. Builds constructive working relationships with physicians, social service providers and others by involving them in the Care Coordination process, and assists in problem resolution involving members and providers, including information for the member grievance process.
  4. Reviews, monitors and updates care plans as indicated
  5. Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services. Provides referrals for community resources and social services as necessary.
  6. Documents member information and contacts made regarding member's care and services provided. 
  7. Actively monitors member's ability and willingness to engage in treatment regimes, and problem solves with staff, members and providers, alternatives or solutions to their needs. 
  8. Educates members on preventative health guidelines and programs.


Experience and Skills:

  1. Bachelor’s degree in social service, rehabilitation, psychology, other related degree., or equivalent experience.
  2. Related health care or social work experience.
  3. A personal vehicle, valid State of Wisconsin motor vehicle operator’s license and conformity with insurance coverage limits are required.
  4. Ability to effectively communicate thoughts, ideas, and information both orally and in writing.
  5. Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects. Ability and willingness to change priorities when necessary.
  6. Strong interpersonal skills and ability to effectively interact with members and coworkers from a variety of different backgrounds and experiences in a professional, diplomatic and courteous manner.
  7. Ability to work effectively as a team member and cooperate in achieving company goals.
  8. Knowledge of physical and mental impairments and related medical conditions.
  9. Interest and willingness to travel to all locations of Milwaukee County to serve persons with severe disabilities.
