Job Description:

This professional position is responsible for overseeing the disease management of members with chronic behavioral health conditions. The Behavioral Health RN Case Manager works with providers of health care and social services to meet identified complex and episodic mental health, AODA, medical and social needs of Independent Care members and to promote the efficient and cost-effective delivery of services.

  1. Assists in completion of in-home initial Specialty assessments for behavioral health and AODA conditions.
  2. Assists Care Managers in the development and oversight of care plans and prioritization of complex behavioral health, AODA, medical, and social needs.
  3. Monitors and directs members to appropriate specialty services and alternative treatment settings.
  4. Monitors inpatient activity for outcomes related to utilization, readmission, quality of care, and provider performance in compliance with iCare policies.
  5. Collaborates with iCare’s RN Discharge Planners to obtain information on daily inpatient stays for members enrolled in the Disease Management program, assists in the discharge planning process if needed, and provides updates to Care Managers as appropriate.
  6. Coordinates services, communicates information, and interfaces with health care providers regarding the management of member’s current behavioral health, AODA, medical, and social needs.
  7. Interprets and administers benefits in accordance with regulatory and contractual requirements.
  8. Actively problem solves with staff, members and providers in finding alternatives or solutions to their behavioral health, medical, and social needs.
  9. Provides accurate and timely documentation per iCare’s work processes and guidelines.
  10. Contributes to interdisciplinary iCare team meetings as either a participant or a facilitator.
  11. Practices and promotes safety in the workplace.
  12. Acts as a resource for iCare staff and assists with training employees.

Experience and Skills:

  1. Must be a Registered Nurse, licensed in the State of Wisconsin with at least two years of health care or related experience.
  2. Ability and willingness to work in the community and directly with the members when necessary (i.e., home, shelter, facility).
  3. Demonstrated ability to work independently with minimal supervision and to make independent decisions addressing member needs.
  4. Expert ability to handle unpredictable and complex behavioral issues.
  5. Interest and practiced ability to provide direct intervention to facilitate behavior changes in members. Knowledge of behavioral modification techniques used to assist the member in making necessary behavioral changes for optimal health outcomes.
  6. Strong demonstrated interpersonal skills and ability to make high-quality decisions on complex client issues.
  7. Working practical knowledge of Medicaid and Medicare.
  8. Ability to use a personal computer and capability of learning software applications of Independent Care.
  9. A personal vehicle, valid State of Wisconsin motor vehicle operator’s license and conformity with insurance coverage limits are required.
  10. Must be able to travel to any location within Milwaukee County. Additional travel outside of Milwaukee County may be required.
