Job Description:

This professional position provides mental health and/or substance abuse consultative services to Care Coordination for SSI members. The Intervention Specialist delivers short-term, high intensity Wisconsin Interdisciplinary Care Team (WICT) case management with the goal of providing member education and eliminating or improving non -adherence to treatment.

  1. Work collaboratively within the Care Coordination Department to identify members with risking risk/acuity who can benefit from more intensive mental health and substance abuse consultative services.
  2. Identify the appropriate treatment needs, level of service and referral sources for members with psychiatric and substance abuse diagnoses and facilitate their implementation.
  3. Collaborate with and serve as a resource for other iCare departments.
  4. Educates members on preventive health guidelines and programs.
  5. Provide innovative and creative alternatives to assigned members with the objectives of modifying behavioral patterns.
  6. Practice intervention techniques to deliver services that assist member needs.
  7. Build constructive relationships with various social and health services in order to access benefits thereof including Community Support Programs (CSP) and Comprehensive Community Services (CCS) and determine if CSP or CCS can be engaged within the WICT.
  8. Utilize effectively the delivery of ancillary services to meet member needs.
  9. Identify and facilitate the delivery of ancillary services to meet member needs.
  10. Document thorough assessments, interventions including weekly care conference meetings and care plan updates within TruCare following iCare’s Documentation Standards.
  11. Complete projects and reports as assigned.
  12. Review interventions utilized and make modifications as necessary.
  13. Assist in health care cost containment through innovative utilization management practices.
  14. Accepts and performs other related duties and responsibilities as required.

Experience and Skills:

  1. Master’s level degree. Licensed Professional Counselor or Licensed Clinical Social Worker with concentration in behavioral health and substance abuse issues.
  2. At least three years’ experience working in the field of mental health and substance abuse rehabilitation.
  3. Prior working experience with population group that requires greater than normal direction and assistance in modifying behavior and crisis/risk management.
  4. A strong willingness to work as a team member.
  5. Ability to:
    1. demonstrate flexibility and set priorities with daily demands and work assignments.
    2. effectively and professionally communicate thoughts, ideas and information orally and in writing.
    3. handle unpredictable and complex behavioral issues.
    4. work in the community and directly with the members when necessary.
    5. work independently with minimal supervision and to make independent decisions that creatively address member needs.
    6. provide direct intervention to facilitate behavior changes in members.
  6. General knowledge of Medicaid and Medicare.
  7. General working knowledge of Microsoft Office, Excel and Word.
  8. Knowledge of physical and mental impairments and related medical conditions.
  9. Skill and ability to be creative and innovative in meeting client’s needs.
  10. Strong interpersonal skills and ability to make decisions on complex client issues.
  11. A personal vehicle, valid State of Wisconsin operator’s license and conformity with insurance coverage limits are required. Must be able to travel to any location within Milwaukee County.
  12. Ability to use a personal computer and capability of learning software applications of Independent Care.
