Job Description:

This professional position is responsible for implementation, monitoring, and supervision of care coordination activities. The Supervisor of Care Coordination is also responsible for assisting with process improvement and quality issues within the department.

  1. Responsible for the daily oversight and supervision of designated staff within Care Coordination.
  2. Assists in the identification of staff challenges and works with the Managers of Care Coordination to address these challenges.
  3. Assists in development of appropriate standards of performance for employees.
  4. Assists with the audits of assessments, care plan documentation, general documentation and ER/Hospital Follow Up requirements.
  5. Assists in the implementation and monitoring of Care Coordination programs and workflows to ensure alignment with the organization’s quality improvement program.
  6. Assists in the development of policies and procedures to improve and standardize services to members.
  7. Evaluates departmental processes and recommends modifications and improvements.
  8. Monitors systems to achieve the plan’s contracted performance objectives.
  9. As directed by the Manager of Care Coordination, works with other Care Management managers to assure consistent application of the iCare Model of Care.
  10. Provides continued support for the integration of behavioral, social and medical elements of the care management model.
  11. Contributes and participates in department meetings
  12. Serves as a back up to the Managers of Care Coordination.
  13. Participates as chairperson and/or member on pertinent committees and at appropriate meetings.

Experience and Skills:

  1. Bachelor’s degree in social service, rehabililtation, psychology or other related degree.
  2. Previous experience with Medical/Behavioral focused case management and managed care experience.
  3. Previous experience working with persons with disabilities in a healthcare setting.
  4. Previous experience working with Microsoft Outlook, Word, and Excel
  5. Previous supervisor experience preferred.
