Job Description:

This professional position provides guidance and support to Care Coordinators while managing a caseload.

  1. Assists with member/provider social functions and member recreational activities.
  2. Assists with training employees.
  3. Assists with input for program and policy and procedure development and assists with input in the QI/QA program.
  4. Assists Managers and Supervisor with overseeing the day to day office operations.
  5. Assists with monitoring team performance and helps report on metrics.
  6. Assists with quality audits as needed.
  7. Conducts initial and annual assessments of members’ medical, behavioral and social needs.
  8. Coordinates and organizes information from members’ assessment to develop a care plan for that addresses their needs and assures implemented interventions are collaborated with members and their providers/supports.
  9. Builds constructive working relationships with physicians, social service providers and others by involving them in the care coordination process, and assists in problem resolution involving members and providers as a member advocate including information for the member grievance process.
  10. Reviews, monitors and updates care plans as indicated.
  11. Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services. Provides referrals for community resources and social services as necessary.
  12. Documents member information and contacts made regarding member’s care and services provided.
  13. Actively monitors member’s ability and willingness to engage in treatment regimes, and problem solves with staff, members and providers, alternatives or solutions to their needs.
  14. Conducts follow up calls post hospitalization and emergency room visits according to policy.
  15. On a rotational basis, provides after-hours on call coverage for service in accordance with Independent Care policies.
  16. Contributes to interdisciplinary iCare team meetings as either a participant or a facilitator.
  17. Educates members on preventive health guidelines and programs.

Experience and Skills:

  1. One year experience in the Care Coordinator Role
  2. Bachelor’s degree in social service, rehabilitation, psychology or other related degree.
  3. Related health care or social work experience.
  4. A personal vehicle, valid State of Wisconsin motor vehicle operator’s license and conformity with insurance coverage limits are required.
  5. Ability to effectively communicate thoughts, ideas, and information both orally and in writing.
  6. Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects.
  7. Strong interpersonal skills and ability to effectively interact with members and coworkers from a variety of different backgrounds and experiences in a professional and courteous manner.
  8. Ability to work effectively as a team member and cooperate in achieving company goals.
  9. Problem solving ability to seek solutions using appropriate methodologies.
  10. Ability to use a personal computer and capability of learning software applications of Independent Care.
  11. Ability to work independently with minimal supervision.
  12. Will maintain confidentiality regarding Independent Care and its member information.
