Job Description:

SUMMARY:

This professional position provides care management services to meet medical, behavioral health and social needs of Independent Care members while promoting and maintaining the quality of care for members.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Conducts initial and ongoing assessment of members’ (children and their caregivers) medical, behavioral and social needs. 
  2. Coordinates and organizes information from members’ assessment and develops a plan of care to address their needs, with review/assistance as needed by the RN case manager and implements interventions by collaborating with members and providers.
  3. Assist parents/caregivers with obtaining timely immunization and health checks for their children.
  4. Reach out to members due to post hospitalization or ER visit to provide coordination of follow-up care.
  5. Builds constructive working relationships with physicians, social service providers and others by involving them in the Care Coordination process, and assists in problem resolution involving members and providers as a member advocate including information for the member grievance process.
  6. Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services. Provides referrals for community resources and social services as necessary.
  7. Documents member information and contacts made regarding member’s care and services provided in the EHR.
  8. Reviews, monitors and updates the plan of care as indicated.
  9. Actively monitors member’s ability and willingness to engage in treatment regimes, and problem solves with staff, members and providers, alternatives or solutions to their needs.
  10. Contributes to interdisciplinary iCare team meetings as either a participant or a facilitator.
  11. Educates members on preventive health guidelines and programs.
  12. Practices and promotes safety in the workplace.
  13. Fully participates in iCare’s Compliance Program, including compliance with iCare’s Code of Conduct, policies and procedures and all applicable Privacy and Security laws.

Experience and Skills:

  1. Bachelor’s degree in social service, rehabilitation, psychology or other related degree.
  2. Related health care or social work experience working with diverse groups of people, including adults, children and pregnant women.
  3. A personal vehicle, valid State of Wisconsin motor vehicle operator’s license and conformity with insurance coverage limits are required.
  4. Ability to effectively communicate thoughts, ideas, and information both orally and in writing.
  5. Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects.
  6. Strong interpersonal skills and ability to effectively interact with members and co-workers from a variety of different backgrounds and experiences in a professional and courteous manner.
  7. Ability to work effectively as a team member and cooperate in achieving company goals.
  8. Problem solving ability to seek solutions using appropriate methodologies.
  9. Must be able to travel to any location within MilwaukeeCounty.
  10. Ability to use a personal computer and capability of learning software applications of Independent Care.
